Instructor Information Form Divisions must submit the following information to the P&VPAA Office to enter or change instructor information in Banner.
Note: Other hiring paperwork can be obtained from Human Resources (including Advertising Plan, Faculty/Staff Personnel Request, and Job Posting).
Student-Related Forms:
Academic Dishonesty Report Form
New Diploma Request Form: Link to the Registrar's Office diploma request site.
Overload Form: Students need to fill this form out to enroll for greater than 17 credit hours. Come to MC 203 to obtain a signature.
Substitution Form: Link to the Registrar's Office substitution form.
Withdrawal Form: Students who are withdrawing from the university need to come to the Vice President's Office, MC 203, and fill out the withdrawal form in person. If you cannot come in person, send an e-mail to withdraw@truman.edu or a fax to (660) 785-7460 with your name, Banner ID, semester you're withdrawing from, and reason for withdrawal.
